Job Summary
Changing Futures aims to drive sustainable, systemic change for individuals facing multiple disadvantages in Bristol. This role supports the delivery and integration of the My Team Around Me (MTAM) approach across the city, fostering coordinated, multi-agency support for those most in need.
Key Responsibilities

Identify individuals suitable for the MTAM approach via multi-agency case forums (e.g., MASH, CRG).

Support professionals to form and maintain MTAM groups using shared safety planning tools.

Embed trauma-informed, person-centred practices within teams.

Provide access to MTAM training, reflective practice sessions, and collaborative planning resources.

Promote inclusive, recovery-focused solutions and remove systemic barriers through shared expertise.

Coordinate with Community Connectors to enhance support networks and pathways beyond services.
Person Specification
Essential:

Experience in co-producing and developing innovative services.

Skilled in multi-agency collaboration and systems change.

Proven understanding of trauma-informed care and inclusive practice.

Background in working with individuals facing complex issues (e.g., mental health, homelessness, substance misuse).

Ability to work independently across services and support others through reflective practice.
Desirable:

Lived experience of multiple disadvantage.

Understanding of relevant legislation and psychological theories.
